| Q: | What is the last day for data to be entered into the system? |
| A: | All data should be entered for the previous month by the 5th of the following month. |
| Q: | How is data stored into the system? |
| A: | The data is stored on an off-site server at the contractor’s location. |
| Q: | If there are any questions about the On-line system whom do I contact? |
| A: | Questions about the site should be directed to OAD/Research, Evaluation and Information Technology Help Desk at 225-342-3846. |
| Q: | How can I login directly to the on-line Satisfaction and Discharge Assessment Survey form? |
| A: | The Satisfaction and Discharge Assessment Survey form can be directly accessed at (www.oad-reit.com/satisfaction/index.htm). Please add this link to your favorites. |
| Q: | If I can’t enter all my data by the 5th, what should I do? |
| A: | If the data is not entered by the 5th, it will not be counted for that month’s submissions. So it is to your advantage that all data be entered into the system by the 5th of every month. |
| Q: | Can I edit a record after I have submitted it to the system? |
| A: | There is no way to edit records. |
| Q: | How do I delete/remove a client record that has been erroneously submitted to the system? |
| A: | Currently the user does not have rights to delete/remove a record from the database. Contact OAD/REIT if this happens to you. |
| Q: | What should I do if the system is not available? |
| A: | Make sure you complete your paper survey form so it can be entered into the on-line system when it becomes available. |
| Q: | How can I generate reports from this system? |
| A: | At this time reports cannot be generated at your site. Reports will be generated at OAD headquarters and distributed to the regions. |
